Creating a Dynamic Form

To create a new dynamic form button navigate to the Installation portion of the page, scroll down to the area that says "Create Dynamic Forms", and click Create New.


On the next screen, assign a title to your button under "Name this button" and then select the fields you want to be in your form by clicking the boxes under "Add Fields". Once your form is complete, click Save in the upper right corner.

Customizing the Fields on a Dynamic Form

In order to customize a Dynamic Form's fields, navigate to the Installation portion of the page, scroll down to the specific button, then click Edit. There are a number of templates available for Dynamic Forms, to access these click Change under the "Base Templates" section. In the window that opens up, select whichever template you wish to use.



Once a base template has been selected, under "Live preview", you can delete fields by clicking the red "X" beside given fields; you can also rearrange the way field appear by clicking, holding, and dragging a field to where you'd like it to be



Enabling Notifications

To enable email notifications for when a Dynamic Form is completed, navigate to the Installation portion of the page, scroll down to the specific button, then click Edit, click Personalization on the left, scroll down to the "Notifications" area, and select "enable". Below enable enter/delete the email addresses you would like notifications sent to.

 


Allowing a User to Finalize a Form Later

To allow a user to finalize the info in a form later, navigate to the Installation portion of the page, scroll down to the specific button, then click Edit, click Personalization on the left, scroll down to "Finalize later", and select "Enable". One enable is selected you are able to edit the text sent to the users email and the text displayed when a user returns to the form later on.



Enabling Follow Ups

To enable follow ups to completed Dynamic Forms, navigate to the Installation portion of the page, scroll down to the specific button, then click Edit, click Personalization on the left, scroll down to the "Other" section, and check the box beside "Answer Follow Up".

Customize a Button's Appearance

To edit the appearance of the Dynamic Form button, navigate to the Installation portion of the page, scroll down to the specific button, then click Edit, click Personalization on the left, scroll down to the "Personalization" section, and under the "Enter image URL" enter the desired image's URL.



Installing a Dynamic Form

To install the button linked to your Dynamic form, navigate to the Installation portion of the page, scroll down to the desired button, and click the Install button beside it. On the menu you that appears there are HTML and iFrame lines of code that can be copied and pasted onto a website and a direct URL that goes straight to the form.
